In order to meet the 1,000 word GE writing requirement, the written assignment for students will consist of a
behavior change paper. The paper will be approximately 7 pages in length with a minimum of 15 references (5
must be primary peer?reviewed references), fully referenced in APA format. The Behavior Change Paper is due
October 27. (No hardcopy papers will be accepted, only online submission via Blackboard)
? No references (bibliography) included with paper will be deducted 10 points
? No references cited in paper text will be deducted 10 points
? Papers that are not in APA format will be deducted 10 points
The written assignment is worth 50 points and will be graded using the ?Written Communication Rubric? and is
available on Blackboard (see Rubrics folder). Plagiarized papers will be reported to the university for disciplinary
action (you cannot submit the same paper to two  different classes). Any student caught plagiarizing a paper
will automatically receive an “F” in the course.)
The paper will consist of the following components:
I. ?How Healthy Are You?? Assessment (1 page) ? Students will need to use the results of their assessment to
discuss why they selected the health issue as their behavior change project (see pg. 20?22 for the ?How Healthy
Are You?? assessment). Remember to set ONE realistic goal that you can achieve throughout the semester (i.e.
go to bed by 9PM, exercise 3 times per week, quit drinking sweetened beverages, keep a schedule to improve
time management and reduce stress, etc.).
II. Background of the problem (2 pages) ? This section introduces the ?health issue? and provides research
related to the behavior change health problem you are trying to improve and must be written ??????in the 3rd
person. Students will review the scientific literature to provide an understanding of the issues related to the
problem, including how many people are affected or die from the health issue, the nature of the problem (signs,
symptoms, potential complications, etc.), and potential risk factors.
III. Behavior Change (2 page) ? Students will describe their behavior change plan. Use the SMART System to
set a realistic behavior change goals that can be achieved over the course of the semester (see Steps One, Two,
and Three on pgs. 14?20). For example, if your behavior change is to: ?lose weight?, describe how much weight,
your weight loss program (i.e., diet plan or exercise plan), and your short term/weekly goals.
IV. Results (2 page) ? Students will describe how they measured their success. Where they able to follow their
proposed plan? Did they make changes to their weekly goals? If so, what did they alter? What were the
successes and/or challenges? Did they reach their ultimate behavior change goal? If the goal was not met, what
were the barriers encountered along the way, what strategies were used to overcome the challenge(s).
This outline will help you to get better idea on what to write about behavior change routines.
1.How healthy are you?
On page 22 I did this assessment called how healthy are you. I scored less on physical health area so I decided to
work on it. One realistic goal that I want to achieve throughout the semester is working out atleast 4 times in a
week. Also, I want to work on my sleeping schedule so I will work on it too by trying to go to bed early and get 7?8
hours of sleep every night.
2. Background of the problems? I will research on the diseases that we could face if not able to get enough sleep
and not able to workout.
3. Behavior change plan
V. My ultimate behavior change goal is to improve my physical health.
VI. One realistic behavior change goal is working out 4 times in a week.
VII. From SMART system I chose Realistic because now I don?t really exercise so I want to start exercising 30
minutes in a day.
4. Results? After working on my goal I will be sharing my views on how I did and what problems I faced during the
accomplishment of my goal.
